fa ارزیابی ارزش تشخیصی فاصله استرنومنتال به عنوان تنها پیشگویی کننده لارنگوسکوپی مشکل در جمعیت ایرانی پزشكي داخلی Internal Medicine پژوهشی Original زمینه و هدف : پیش بینی نمای لارنگوسکوپی و انتوباسیون دشوار از مسایل مهم در بیهوشی می باشد. در این مطالعه ارزش تشخیصی اندازه گیری فاصله استرنومنتال مورد بررسی قرار گرفته است. روش بررسی: در این مطالعه مقطعی، در 500 بیمار 15 تا 65 ساله که تحت بیهوشی عمومی قرار گرفتند فاصله استرنومنتال، انتوباسیون و نمای لارنگوسکوپی دشوار تعیین شد و ارتباط فاصله استرنومنتال با سایر متغیرها بررسی گردید. یافته ها: میانگین فاصله استرنومنتال،16/7±2/3سانتی متر بود. فاصله استرنومنتال با جنس(001/0<P)، سن (001/0<P)، نمای لارنگوسکوپی دشوار (گرید III و IV) ( 001/0 P<) و انتوباسیون دشوار (001/0<P) رابطه معنادار آماری داشت.اندازه 15/5 سانتی متر به عنوان بهترین cut off point تعیین شد. ارزش تشخیصی فاصله استرنومنتال در پیش بینی لارنگوسکوپی و انتوباسیون دشوار به ترتیب حساسیت:2/79 ،4/77 ، ویژگی : 1/68 ،7/68 ، ارزش اخباری مثبت : 1/11 ، 14 و ارزش اخباری منفی : 5/98 و 9/97 داشت. نتیجه گیری: اندازه‌گیری فاصله استرنومنتال به عنوان یک شاخص در پیش‌بینی انتوباسیون دشوار یا لارنگوسکوپی کمک کننده است ولی کافی نیست. Background and Aim:Predicting difficult laryngoscopic view and intubation is an essential problem in anesthesiology. In this study diagnostic value of stemomental distance measurement is assessed. Materials and Methods:In this cross sectional study, in 500 patients 15 to 65 years old who were planned for general anesthesia, stemomental distance, difficult larangoscopic view, difficult intubation and Association of stemomental distance with variables were determined. Results: Mean (±SD) stemomental distance was 16.7±2.3 cm. Stemomental distance was significantly correlated with sex (p< 0.001), age( p< 0.001) , difficult laryngoscopic view (III or IV grade) ( p<0.001) and difficult intubation ( p< 0.001). 15.5 was the selected cut point for stemomental distance. The diagnostic value of stemomental distance in predicting difficult laryngoscopic view and intubation were: sensitivity: 79.2, 77.4 specificity: 68.1, 68.7 positive predictive value: 11.1, 14negative predictive value: 98.5, 97.9, respectively.
