بررسی میزان ایمونوگلبولینهای IgA، IgM و IgG در کمخونی ناشی از فقر آهن

زمینه و هدف: ایمونوگلبولینها از دسته گلیکوپروتئینها هستند که در سرم و مایعات بافتی تمام پستانداران وجود دارند و در میان پنج کلاس ایمونوگلبولینها در انسان، سهکلاس IgG، IgA و IgM در مقابل عواملبیگانه از اهمیتبیشتریبرخوردارند. مطالعهحاضرباهدف بررسیمیزان ایمونوگلبولینها در کمخونی ناشیاز فقرآهن انجامشد.
روش بررسی: در این مطالعه توصیفی- تحلیلی و مورد- شاهدی، 90 نفر از مراجعهکنندگان به آزمایشگاه مرکزی یزد که دارای علائم کلینیکی کمخونی بودند و فریتین آنها از کمتر از mg/dl 10 بود، مورد تحقیق قرار گرفتند (گروه مورد)؛ این افراد هیچ بیماری مشخص دیگری نداشتند و در 4 ماه گذشته تزریق ایمونوگلبولین یا واکسن انجام نداده بودند؛ همچنین 90 نفر مراجعهکننده که سالم و فاقد علائم کلینیکی کمخونی بودند و از نظر سن و جنس با افراد گروه مورد هماهنگ بودند، به عنوان گروه شاهد، انتخاب شدند. از افراد هر دو گروه به میزان cc5 خون وریدی گرفته و داخل لوله ساده ریخته شد؛ 4 ساعت بعد سرم آن جدا و در فریزر در دمای C80- نگهداری شد؛ ایمونوگلبولینهای IgG ، IgM و IgA به روش انتشار شعاعی ساده (SRID) و مقدار فریتین آن بهروش ELISA اندازهگیری شد. اطلاعاتبا استفاده از آزمونهایآماری Chi-Square و t مورد تحلیل قرارگرفتند.
یافتهها: از 90 نفر افراد دارای کمبود فریتین، 17 نفر (88/18%) در گروه سنی 1-19، 43 نفر (77/47%) در گروه سنی 20-29سال و 30 نفر (33/33%) در گروه سنی 30-59 سال قرار داشتند. میانگین IgG در افراد گروه مورد mg/dl 33/877 و در افراد گروه شاهد mg/dl 7/1048 و اختلاف آنها از نظر آماری معنیدار بود (001/0=P). میانگین IgM در افراد گروه مورد mg/dl 2/134 و در گروه شاهد mg/dl 92/138 بود؛ این اختلاف از نظر آماری معنیدار نبود (487/0=P). میانگین IgA در گروه مورد mg/dl 82/149 و در گروه شاهد mg/dl 33/179 و اختلاف آنها معنیدار بود (002/0=P). تفاوت وضعیت ایمونوگلبولینها در دو گروه مورد و شاهد از نظر IgG و IgA معنیدار بود (به ترتیب 000/0=P و 003/0=P) ولی از نظر IgM معنیدار نبود (756/0=P). میزان همبستگی فریتین با ایمونوگلبولینها در هیچ از یک گروههای مورد مطالعه معنیدار نبود.
نتیجهگیری: در این تحقیق، میانگین ایمونوگلبولینهای IgG و IgA در افراد با کمبود فریتین به طور معنیداری کمتر از افراد با فریتین نرمال بود.ن
Background and Aim: Immunoglobulins or antibodies are a group of glycoproteins found in the serum and lymphatic fluids of all mammals. They are produced in large amounts by plasmocytes which originate from B lymphocytes. Of the five classes, of human immunoglobulins, three classes including IgG, IgA and IgM are of greater importance against foreign bodies. They play an important role in controlling infectious factors. The aim of the present study was to evaluate the rate of immunoglobulins in iron deficiency anemia.
Materials and Methods: In this case-control study, 5cc of venous blood were obtained and poured into the single test tubes from both case subjects with the clinical signs of anemia without any other disease and control healthy subjects of the same age and sex referred to Yazd central laboratory. After 4 hours the serum was separated and kept in freezer at 80C. The immunoglobulins IgG, IgM and IgA were measured using the SIRD method. The amount of ferritin was measured using ELISA. Statistical data were analyzed using chi-square and t tests.
Results: Out of 90 subjects with ferritin deficiency, 17 subjects (18.88%) were in the age group of 1-19 years old, 43 subject (47.77%) were in the age group of 20-29 years old and 30 subjects (33.33%) belonged to the age group of 30-59. The average of IgG in the subjects with ferritin deficiency was 877.33 mg/dl, while for the subjects with normal ferritin level was 1048.7 mg/dl showing a statistically significant difference (P=0.001). The average IgM in subjects having ferritin deficiency was 134.2 mg/dl while in the subjects with normal feretin level it was 138.92 showing no statistically significant difference (P= 0.487). The average IgA in subjects with ferritin deficiency was 149. 82 mg/dl while it was 179.33 for the subjects with normal ferritin level showing a statistically significant difference (P= 0.002). The correlation coefficient of ferritin with immunoglobulins was not statistically significant in the subjects with ferritin deficiency and the subjects with normal ferritin levels.
Conclusion: The average of IgA and IgG in the subjects with ferritin deficiency was significantly less than that of subjects with normal ferritin.
