تأثیر تمرینهای کششی و موبیلیزاسیون عصب در بهبود درد و عملکرد اندام فوقانی در افراد با سندرم خروجی قفسه سینه
The effect of stretching and nerve mobilization exercises on improvement of pain and upper extremity function in subjects with thoracic outlet syndrome

زمینه و هدف: درمان سندرم خروجی قفسه سینه مشکل چالش آوری برای فیزیوتراپیست ها است و درمان محافظه کارانه به عنوان راهکار ترجیحی برای این بیماران مورد تاکید قرار می گیرد. هدف از این مطالعه مقایسه تأثیر تمرینهای کششی و موبیلیزاسیون عصب بر شدت درد و عملکرد اندام فوقانی در مبتلایان به سندرم خروجی قفسه سینه بود.
روش تحقیق: برای این کارآزمایی کنترل شده تصادفی دو سوکور20 بیمار مبتلا به سندرم خروجی قفسه سینه با میانگین سنی 9/12±95/34 سال از کلینیکهای فیزیوتراپی زاهدان انتخاب شدند. بیماران بصورت تصادفی در دو گروه تمرینهای کششی (10مورد) و موبیلیزاسیون عصب (10مورد) قرار گرفتند. دامنهی حرکتی با گونیامتر، شدت درد با مقیاس چشمی درد (VAS)، حس تمایز دو نقطه بوسیله پرگار و عملکرد شانه با مقیاس شاخص رتبهای درد و ناتوانی شانه قبل و بعد از سه هفته درمان اندازهگیری شدند. از آزمونهای t مستقل و زوج جهت مقایسه نتایج بین و درونگروهها استفاده شد.
یافتهها: میانگین نمره شاخص درد و ناتوانی شانه در گروه تمرینهای کششی از 6/9±3/92 به 9/13±5/40 و در گروه تمرینهای موبیلیزاسیون عصب از 3/16±0/81 به 06/12±0/25 کاهش پیدا کرد (٠5/0>p). کاهش میانگین نمره شاخص درد و ناتوانی شانه در گروه تمرین های موبیلیزاسیون عصب نسبت به گروه تمرین های کششی بیشتر بوده است (٠5/0>p). میانگین دامنهی حرکتی مفصل شانه بعد از درمان در هر دو گروه افزایش نشان داد (٠5/0>p). تفاوتی بین دو گروه از نظر دامنهی حرکتی مفصل شانه، نمره شدت درد و مقادیر تمایز دو نقطه دیده نشد (٠5/0<p).
نتیجهگیری: هر دو نوع تمرینهای کششی و موبیلیزاسیون عصب سبب کاهش درد، بهبود حس تمایز دو نقطه دست و بهبود عملکرد و دامنه حرکات مفصل شانه میشوند. اما تمرینهای موبیلیزاسیون عصب در بهبود عملکرد اندام فوقانی نسبت به تمرینهای کششی مؤثرتر هستند.
Background and Aim: Treatment of thoracic outlet syndrome is a challenging problem for the physiotherapists and the conservative approach preferred for treatment. The purpose of this study was to determine and to compare the effects of stretching and nerve mobilization exercises on pain intensity and upper extremity functions in patients with thoracic outlet syndrome.
Materials and Methods: For this double-blind, randomized controlled trail, 20 patients with thoracic outlet syndrome aging 34.95±12.9 years were recruited from zahedan physiotherapy clinics. The patients were randomly assigned to either a stretching (n=10) or a nerve mobilization groups (n=10). Before and after three weeks treatment, shoulder range of motion (degree), pain intensity (ordinal), two point discrimination(mm) and shoulder function (ordinal) were measured using goniometer, visual pain scale, caliper and shoulder pain and disability index, respectively. Independent and paired T-tests were used for comparisons between and within groups, respectively.
Results: The mean of shoulder pain and disability index decreased from 92.3±9.6 to 40.5±13.9 in the stretching exercises group and from 81.0±16.3 to 25.0±12.6 in the nerve mobilization group (P>0.05). After treatment, the mean of shoulder pain and disability index decrement was greater in the nerve mobilization group than the stretching one (P>0.05). The mean range of motion of shoulder increased in both groups (P>0.05). However, no significant difference was seen between two groups in measures of shoulder joint range of motion, pain intensity and two point discrimination (P<0.05).
Conclusion: Both the stretching and nerve mobilization exercises decrease pain intensity and improve hand two point discrimination, shoulder function and joint ranges of motion. However, in order to improve shoulder function, the nerve mobilization exercises are more effective than other one.
